Insurance Investigations
Insurance investigations for carriers, TPAs, and defense counsel
Cosley & Cosley is a Florida investigative firm built around a single client base: the people who pay, defend, and litigate insurance claims. Every assignment is documented to be used — in a claim file, a mediation, or a courtroom.
What an insurance investigation covers
An insurance investigation is the independent, documented verification of what a claim asserts. Depending on the claim, that means observing and recording a claimant's actual physical activity, confirming or refuting the mechanism of loss, locating and interviewing people who saw what happened, or establishing what the scene looked like before evidence disappears.
We do not deliver conclusions or opinions on compensability. We deliver the observable facts, the video and photographs behind them, and a report clean enough to attach to a file or produce in discovery.
- Manned and covert surveillance with time-stamped video
- Claims investigation and activity checks
- Recorded statements from claimants and witnesses
- Scene canvassing and third-party camera recovery
- Social media and open-source documentation with data preservation
- Locates, skip trace, and hospital canvassing
Who assigns this work
Our assignments come from claims adjusters and claims managers, SIU analysts, insurance defense attorneys, third-party administrators, and self-insured employers. Each of those roles needs something slightly different from the same field work, and our reporting reflects that: an adjuster needs a fast, factual activity summary; defense counsel needs a chain of documentation that survives cross-examination.
Coverage and pricing
We cover the entire state of Florida at the same flat rates, with investigators based in Pensacola, Panama City, Jacksonville, Orlando, Melbourne, Tampa, Jupiter, Ft. Lauderdale, and Miami. There are no travel add-ons, mileage lines, or expense surprises. Through reciprocity we can also support cases in southern Georgia and Alabama.
Frequently asked questions
- How quickly can an insurance investigation be assigned?
- Send the assignment through our case submission form or by email. We review the details, set the investigative approach, and Kasey sends a written confirmation of assignment. Most surveillance assignments can be staged within a few business days, and rush requests should be flagged in the submission.
- What information do you need to start?
- At minimum: the subject's name, last known address, date of birth if available, the claim or file number, the type of investigation requested, and the objective. Photographs, vehicle information, employment details, and prior surveillance history make the first day materially more productive.
- Do you work directly with defense counsel as well as the carrier?
- Yes. Assignments frequently come from the carrier and are reported to both the adjuster and defense counsel, or come directly from the firm. Tell us the distribution you want at assignment and we follow it.
